No! Oregon is a direct-access state. You can come directly to us for physical therapy without a referral from your doctor.

We accept most major insurance plans, including Medicare, Medicaid/OHP, Blue Cross Blue Shield, Aetna, Cigna, Pacific Source, Moda, Providence, TRICARE, and VA. Don’t see your plan listed? We may still accept it — call us at (541) 306-1099 and we’ll verify your specific coverage.

Please bring your photo ID, your insurance card, and any relevant imaging or physician’s notes if you have them. Wear comfortable clothing that allows easy access to the area being treated. When you schedule, we’ll email your intake paperwork so you can complete it online ahead of time from your phone or computer, which keeps your first visit focused on you rather than forms. If you’d rather fill everything out in person, that’s perfectly fine — just arrive 15 minutes early and we’ll have your paperwork ready.

Each appointment is typically 45-60 minutes of one-on-one time with your physical therapist. No aides, no shared appointments.

Yes! We offer a free 30-minute in-person consultation and a free 20-minute phone consultation with a physical therapist.

Your physical therapist will prescribe an individualized plan of care during your evaluation, based on your condition, goals, and clinical findings. Many patients experience significant improvement within 6–8 visits.
